Policies & FormsAt Warren Primary Care patients are seen by appointment. In most circumstances, we can accommodate same-day appointments scheduled via phone call. If staff members are assisting patients and you receive our voicemail, leave your name, date of birth, two telephone numbers where you may be reached, and a brief description of your medical problem, so that we may return your call promptly.

Remember to bring your insurance card and a list of all your medications to your appointment.

Medical Records
Your records are confidential and will not be released without signed consent from you.

Please call our office to have patient forms mailed to your home prior to your visit.

We request 48 hours to process referrals, except for emergencies.

Prescription Refills
Refill requests for medications other than controlled drugs may be requested through your pharmacy, which will submit the request to us.  We require 48 hours to process refill requests. Medication refill requests must be made during normal business hours.  Please remember that in certain situations, appropriate medical care requires a patient examination before a prescription can be filled.

For emergencies, our physicians are available 24 hours a day at 908-598-7970.  After business hours you will be transferred to our on-call physican service.  If you are experiencing a life-threatening emergency, call 911.

Insurance and Payment
If you require a co-payment at the time of your visit, we accept cash, checks, Visa and MasterCard.  If needed, patients may contact our billing office at 973-656-6280 to discuss options for financial assistance or payment plans.
